Classical (two-stage) dental implantation with delayed loading

Classical implantation is also called delayed loading. The method refers to a two-stage protocol: in the first stage, an implant is installed, in the second, after 2-6 months, a crown or prosthesis is fixed. Prosthetics are performed only after the artificial root has completely engrafted. The technique is used in the absence of one tooth to the entire dentition. If there is a lack of bone tissue (95% of cases), osteoplasty is performed as a separate preliminary stage or simultaneously with the installation of implants. The advantages of the classical method are strong fixation, aesthetics, and guaranteed results.

Indications

Classical implantation is used in the absence of:

  • one tooth, including the outer ones in the row;
  • several teeth separately or in a row;
  • all teeth in the jaw.

For single defects, one implant with a crown is installed. For long-term defects, each tooth can be restored with a separate implant with a crown or a smaller number of artificial roots and a bridge can be used. If all teeth are missing, you need to install at least 6-8 implants and a fixed denture with 14 crowns or 4 implants and a removable denture.

When is bone grafting needed?

Around the artificial root, a bone reserve of 2-3 mm in width is needed for strong fixation. And the lack of height of the alveolar ridge leads to unreliable fixation of the implant or excessive immersion of the structure.

Indications for osteoplasty:

  • After tooth extraction, the jaw bone atrophies, the gums and jaw resemble a thin ridge.
  • Congenital anatomical deficiency of bone tissue.
  • Cell destruction, thinning of gums and bone as a result of inflammation.

Stages of implant installation

  • Preparation

    The doctor examines the patient to identify contraindications. Based on three-dimensional images, evaluates the structure, density and size of the jaw. Sanitation of the oral cavity is mandatory - includes the removal of tartar, treatment of caries, and elimination of gum inflammation. Depending on the identified obstacles to implantation, the preparatory stage lasts from a couple of days to 1 month. After tooth extraction, the implant is placed after 2 months, which is necessary for the socket to heal and the bone to recover. If there is not enough bone tissue, osteoplasty is performed, which extends the preparatory stage to six months.

  • Implant installation (first surgical stage)

    It is performed under local anesthesia or sedation. The structure is implanted into the bone, exfoliating part of the gum. A dental instrument is used to form a bed into the bone for the structure. Then a plug is attached to the artificial root and the gum is sutured. The operation lasts from 20 to 40 minutes. The implant on the lower jaw takes 2-4 months to take root, on the upper jaw - up to six months. During osseointegration, the patient is fitted with immediate butterfly prostheses. They do not injure the established root and replace teeth, attached to adjacent hooks.

  • Installation of the gum former (second surgical stage)

    The gum is opened, the implant plug is removed, and the former is installed. The operation is simple, performed under anesthesia and lasts 15 minutes. It takes 10-14 days to create an aesthetic gum contour.

  • Prosthetics

    The former is replaced with an abutment, which serves as a support for the crown and connects the implant to the orthopedic structure. The doctor makes impressions. After a maximum of a week, the manufactured crown is fixed to the artificial root.

What implants and crowns do we install?

At the Dr. Levin dental center, only Nobel Biocare implants are used. We chose this system as one of the best - the implant survival rate is 99.3%.

Artificial roots are made of medical purified titanium. The surface with a porous structure of Ti-Unite in its chemical composition resembles a human tooth root, so the structures quickly take root. After 2 weeks, the jaw cells grow together with the installed artificial root without titanium oxidation. When manufacturing implants, vacuum casting technology is used without milling. The finished models are lightweight and therefore do not cause increased pressure on the bone.

For prosthetics, we use only metal-free crowns and bridges made using the Procera® method from Nobel Biocare or in our own digital production using Zirkonzahn Schule technology. We abandoned metal-ceramic prostheses a long time ago.

Metal-free crowns made of ceramics and zirconium dioxide are compatible with soft tissues, retain their shade during use, do not cause discomfort, and look like natural teeth.

Bridges consist of zirconium crowns and a titanium arch that unites the structure. To fix dentures on implants, Multi-unit abutments from Nobel Biocare are used.

Price - what is included and what the cost depends on

The final cost of treatment depends on:

  • Number of implants installed.
  • Implant models. The Nobel Biocare PMC implant for weak bone differs from the Nobel Biocare Conical Parallel CC models for dense bone in the purchase price and installation time.
  • Types of anesthesia. Local or sedation is used.
  • Sinus lift and bone grafting methods. Open sinus lift is more expensive than closed sinus lift. The price of osteoplasty is determined by the consumption of the regeneration stimulator and depends on the choice of osteoplastic material.

Advantages and disadvantages of the classical method

  • reliable fixation of implants;
  • suitable for replacing one tooth, partially missing teeth and the entire dentition;
  • achieving aesthetics through the installation of individual abutments and the formation of a gingival contour;
  • installation of crowns and bridges without artificial gums;
  • The dentures do not cause discomfort and are held securely.

Minuses

  • duration of treatment;
  • the need for bone grafting;
  • risk of tissue inflammation.

Temporary prosthesis

After 3 - 6 months, when the doctor is convinced that the implant is stabilized in the bone, a gum former is installed on it, since the gum line, due to the lack of a crown, may noticeably drop during this time. After two weeks, the former is removed, an abutment is installed and a temporary prosthesis is secured.

Comparison of two-stage and one-stage implantation

The second name for the one-stage protocol is implantation with immediate loading. Installation of the implant and fixation of the crown occurs in one stage - in one day (maximum 2-3 days), there is no need to wait for the implantation of the artificial root.

Temporary metal-plastic crowns are immediately installed. They are made lower in relation to the adjacent teeth (removed from the bite) in order to avoid displacement of the artificial root during chewing. After the implant has fused with the jawbone, the temporary crowns are replaced with permanent ones made of zirconium dioxide.

The method allows you to quickly restore aesthetics and, to some extent, chewing functions, but has limitations:

  • there must be enough bone to securely fix the implant;
  • Only those teeth that do not take an active part in chewing can be immediately loaded with a crown.

Contraindications

The use of this technique is not indicated in all cases; contraindications include:

  • oral hygiene is poor, there are inflammatory processes;
  • multiple caries lesions;
  • bite pathologies;
  • alcohol abuse, smoking;
  • pregnancy at any stage;
  • mental disorders;
  • blood clotting disorders;
  • pathologies of metabolism, immune system functioning;
  • tuberculosis, lung diseases in the acute stage;
  • diabetes at any stage;
  • hypertonicity of the jaw muscles.
